How do I start my claim?
You start your claim on or after the day of your surgery, on the EDD website ( https://edd.ca.gov/sdi_online ). Once you have
filed and paid our office we will submit the practitioner’s portion on or after the day you are released from the hospital.

When does disability start?
We typically start your disability on your surgery date, releasing you back to work the day after your one month follow up
appointment. Family members are only allotted two weeks off from surgery date. We do not approve intermittent time off.
Any changes to disability start date or any extensions must first be approved and notated by a provider.

SDI Online provides claimants, physicians/practitioners, and employers with electronic and automated options that are simple to use and available 24 hours a day, 7 days a week.

How Do I File a New Disability Claim on Paper?
Disability Insurance claims may be filed on paper, through our office. You will pay the $40 filing fee and pick up the red SDI form at the front office to fill out by hand, HOWEVER, it is less expensive to file online.
How Do I Submit my FMLA, Accommodation or any other work forms?
If you already have your forms in hand, you can give them to the front desk and they will forward them to our Forms department.
If your Employer is requesting a fax number, they can fax the forms to Central California Surgery directly (209)846-9641.
